i noticed after i started getting disability for my mental health - that when i went to hospitals they stopped putting my diagnosis on the sheets. idk if its cause i have disability or if they just didnt do it. two times i got no diagnosis on the discharge papers. but 1 time i saw it on another paper but it was on the discharge paper.

whats the reason?

Sometimes they'll just write the code # for dx instead of naming it out.

I have had to ask what the code numbers mean re my dx's.

If there are neither, ask your pdoc, nurse or even receptionist.
