I need help asap filling out a form and answering the question of what I do all day. I know it should be easy to answer and I know the answer. The problem isn't that. I don't understand, but I'm having panic attacks every time I try to fill this out. And I have this weird feeling that's hard to describe that proceeds acute stress responses.

I just need a therapist to help me fill out that form without having a mental health disaster. It doesn't even need to be a good one. Anyone? PMs are welcome.

Have you tried looking for a local office of the Mental Health Association? They generally are low or no fee and provide all types of support, including paperwork and finding services. Here is a link to their website on the national level. You can search for local offices on the site:
