So my psychiatrist prescribed me the Emsam patch yesterday (it's an MAOI), and then promptly told me he knows nothing about it and I might want to research it....
And he refuses to write a summary letter for me that I have requested twice now so I can go to the mayo clinic.... Gotta love it!! ![]() |

Yeah, I'm not sure if I should start with the Emsam or not. I'm worried that if I have any kind of reactions/side effects, I have nobody to call, since my doctor wouldn't have a clue what to do....

My pdoc put me in the hospital to monitor me while he put me on the ensam patch. That's how wary he is of this med. Watch the blood pressure. That can be a real problem. In my case it made my blood pressure drop to the point that I was passing out repeatedly (needless to say I'm not on ensam anymore).

![]() Oh wow. That is totally unacceptable. I would fire him and find a better doc pronto. FWIW I was on Emsam patch for a year before I switched to oral. I have not had any problems with either. The patch does not seem to be a problem with dietary tyramine the way oral med does.

I am tring to get in with a pyschiatrist at the Mayo clinic, but that may take some time (especially since my current doc won't write the letter I need him to write), so in the meantime, I am stuck with Dr. Stupid.
I have read that you do not need to make any dietary changes with the 6mg Emsam patch, which is what I will be taking, if I decide to start it, so hopefully that will work... |

nicoleb, why can't you go to the Mayo Clinic without the letter from your pdoc?
One of the most important things to know about emsam (or other forms of seligiline) is do not take if you are also taking opioids or tricyclic ADs. Opioids include morphine, demerol, oxycodone, vicodin, etc. Good luck.

Just a small list of what not to take with it:
Tell your doctor if you are taking, have recently taken, or plan to take any of the following prescription and non-prescription medications, herbal products, or nutritional supplements: amphetamines (stimulants, 'uppers') such as amphetamine (in Adderall), benzphetamine (Didrex), dextroamphetamine (Dexedrine, Dextrostat, in Adderall), and methamphetamine (Desoxyn);antidepressants such as amitriptyline (Elavil) and imipramine (Tofranil); buproprion (Wellbutrin, Zyban); buspirone (BuSpar); carbamazepine (Tegretol); cyclobenzaprine (Flexeril); dextromethorphan (Robitussin); medications for cough and cold symptoms or for weight loss; meperidine (Demerol); methadone (Dolophine); mirtazapine (Remeron); other monoamine oxidase inhibitors such as isocarboxazid (Marplan), phenelzine (Nardil), oral selegiline (Eldepryl, Zelapar), and tranylcypromine (Parnate); oxcarbazepine (Trileptal); pentazocine (Talwin); propoxyphene (Darvon); selective serotonin reuptake inhibitors such as citalopram (Celexa), escitalopram (Lexapro), fluoxetine (Prozac), fluvoxamine (Luvox), paroxetine (Paxil), and sertraline (Zoloft); selective serotonin and norepinephrine reuptake inhibitors (SSNRIs) such as duloxetine (Cymbalta) and venlafaxine (Effexor); St. John's wort; tramadol (Ultram, in Ultracet); and tyramine supplements. Your doctor may tell you not to use transdermal selegiline until 1 or more weeks have passed since you last took one of these medications. If you stop using transdermal selegiline, your doctor will probably tell you not to take any of these medications until at least two weeks have passed since you stopped using transdermal selegiline. From: http://www.ncbi.nlm.nih.gov/pubmedhealth/PMH0000354 I wouldn't touch it with a 10 foot pole. . . or an 11 foot swede.

Know it's very dangerous taking meds without the dr's knowing anything about the side effects. I have been in that situation before, but have alway been in the hospital at the time. I doubt that I would take the med at this point without having a good pdoc backing up my meds. What is his reasoning for refusing to write you the letter you are requesting....is he afraid that you will leave him for another pdoc?. I would talk with your medical state board representative & see if they can apply some pressure for you to get your needed letter....of course, maybe just the threat of you talking to them will cause him to write the letter given his lack of knowledge. We as patients need to stop being made to feel so helpless in the situations that SOME Dr's continually seem to put us into. Wishing you the best in this situation

Dr. Stupid finally wrote me my letter for the Mayo clinic and faxed it to them on Friday so hopefully I will hear from them soon. I decided to go ahead and try the patch, because I really can't stay on nothing at this point. I have not had any probems, so far.
