Hi I had a school meeting today and their asked me about my medication and i am taking an Antipsychotic ablifry and the adult servie who does adult services for people with delayed learning said my Pdoc has to do a form or something for Antipsychotic for some reason ,has anyone ever heard about this. i have to call my Pdoc and explain this to him

hi. the public/community clinic I go to does that once a year. Its an informed consent form...they just need a signature so they don't get sued if something goes wrong.

It could also be a form that you have to give to the pharmacy to get your prescription filled. One of my meds is like that...I have to get a paper from my pdoc and hand-deliver it to the pharmacy, then they fill the prescription. I have no idea why.
