It's been almost 3 weeks now and I'm undecided whether I should make an appointment.... there were no real problems since my last session. Should I still see my therapist?

Do you want to? Can you afford it? If the answer is yes, then why not? I find that knowing it's okay to be doing well and that my therapist is not going to leave me is very comforting.

What do you need from the session?
Sometimes we need to talk over problems or do work on an issue. At other times we just need to maintain the connection with T. Both are valid needs. I have had sessions where my T asks me what work we are going to do and I said "Today I just need to connect with you." He will make a note and then we just talk about life. I have found that these types of sessions usually come before I have breakthrough sessions. I think it is my mind testing the waters to make sure it will be safe before we go in deep.

Sometimes I focus on issues that have come up in the time since the previous session but more often I don't. I try to pick up where I left off the previous week and continue to deal with that topic in depth. I find it easier to do the more interesting work when I'm in a reasonably good space mentally. Otherwise it's just putting out fires and complaining and figuring out how to cope--all of which can be worthwhile but it doesn't really feel like the main reason I'm there.
