If you're an adult, who do they report you to. I never really knew who they were talking about. I know for abuse they would call CPA, but what about suicide or homicide?

What do you mean by "report to"? If you are a legal adult what is said in therapy is confidential, it can't be shared with anyone. If someone thinks you are a danger to yourself they may call police for a well check.

If you won't go to a hospital voluntarily they can call the police to pick you up and take you there.

Police... They can force you to be evaluated or on an involuntary psych hold in the hospital. But not just for "thoughts," more for specific plans.

Homicide, the police
Suicidal thoughts would depend how compelling the thoughts are. But again police if the person was not safe and wouldn't agree to voluntary admission.

Yeah, the police. Or, depending on where you live, the crisis mental health service.
