I was wondering if there is a site where I can do it online? I don't like going into interviews not knowing what is going to be asked of me.
Or if anyone has done it, what all do they ask to know?

It looks dreadfully complicated. I personally wouldn't try to evaluate myself with it.

Do you know that a provider is going to go over all of this with you? In my (albeit somewhat limited) experience, health care providers use much simpler and quicker diagnostic questionnaires (like the PHQ-9) Or just ask you to describe in your own words what the trouble is.

Wow, that does look very complicated! I'm going to take some time to read over it. There will be student giving me this test. My psych is apparently a huge fan of this test

Ouch. Well, try not to over-think it. I know this would drive me NUTS (and possibly contaminate my results).
