My last Psychiatrist left the group and I was passed over to another new Psychiatrist. Our first meeting she seemed concerned, saying I seemed very sad and that we should look at up my medication or trying Ritalin. I said no, but she said we would discuss next appointment. My Psychiatrist appointments are usually 3 mos apart. So my recent appointment she acted very cold, kept asking me what has been going on the past week (a couple of times) I told her about having the withdrawals from Fentynal and that they were really bad so I started using the patches again. She seemed frustrated and told me she didn't know that much about Fentynal. She asked me if I was still on two medications and I told I was on one but not the other. She never mentioned discussing the other meds and acted as if she had nothing else to say. My 30-35 minute appt. turned into a 10-15 appt. So later I looked up her notes where she said she gave me copies of my medication and did the suicide questionnaire and she need neither. Also she mentioned that I was not as jovial as usual. What? I've never been jovial with her. It's like she was talking to someone else. I'm pretty discouraged that this is in my file that I was jovial at some point in my first appointment with her. Should someone else read this they may think I am bs'ing them because it was documented that I was jovial. Should I talk to her about this? It may really tick her off me calling her out on all these errors in my file.

She probably copied and pasted the note from another patients record. That happened to me at the ER just recently. Blatant mistakes and half sentences-you could tell the radiologist copied and pasted from someone elses record. The body features werent mine, and the reason for test was totally wrong. And my treatment was based on the incorrect information.

Called hospital but could never get them to look at images let alone correct the mistakes. I still dont know if they even looked at my results, or if the report was about another patient.

This happens over and over. No one is accountable in health care.

Amazing you have access to your records. I hardly see mine and what i do see is ridden with errors.

Behavior could be related to your taking a controlled substance. Doctors act strange around people taking them, they often want nothing to do with you.

Sorry you had a crappy appointment. Can you ask for a new Dr.? Yes, i would ask her to correct the mistakes.

Thanks, I want to do that but I don't want to upset her. You can "win the battle but lose the war" Glad someone else finds it disturbing and I'm just not over reacting.

I dont blame you. Mine is potentially really serious, so i was really upset about it.

Maybe watch for a big mistake then tell her then. Great you can access your records so quickly. I need to order mine to check them over.

I get my care through the veterans admin. and they post a short post of just the basics, not the full report I'm sure. She said she spent 20-30 minutes with me but it was actually 10-15, my appt. was at 3p.m. and that's when she came to get me, then I was back at the front desk scheduling the next appointment at 3:13 p.m. No way she could have covered everything she said she did. Plus she was constantly typing while we were talking, how rude.

Apparently this is a real thing. I just had a long and very revealing conversation about it with a friend who works in healthcare quality. Pretty amazing stuff. I'm sorry you suffered because of it, Skies.

OP, you could ask your Pdoc about it, but I'd be prepared for the likelihood that she doesn't take it well. I'm sorry this is happening and I hope you find a solution that works for you.

It is unethical what she did. It goes against the hypocratic oath. I would be very wary of any suggestions she makes to you. She is not dialed into your care! You deserve better.

Well it's obvious she didn't have a clue who I was , why I was there and felt it was okay to put something in my file that was not factual from observation. But thanks, I will have to think about this some. Maybe I could get my T to talk to her.

When I worked at the hospital we had at least two confirmation questions we asked or verified with patients to make sure we had the right patient.
