How do I get past the paranoia to have a constructive session?

That's a pretty broad question because it depends on the nature of your paranoia, on you, on your therapist and on your relationship with your therapist. How have you worked with your paranoia in the past and has it been useful? Trying to get past it might paradoxically entail paying attention to it; understanding its nature; what you are trying to communicate to yourself, alongside maintaining a sense of solid reality. The fact that you can identify these thoughts and beliefs as paranoid seems important and can help anchor you in reality.

In times of paranoia I found the session worked as a holding environment until the end of fog cleared.

I tend to try to ignore it.
