how do you tell someone a truth that you know will hurt while making it clear you don't want to hurt just help? or maybe it's best to just not go there?

I think it depends on the situation...is the person who needs help having problems in an abusive relationship or with drugs/alcohol? If so, then I think you need to let the person know that you care and that you don't want to overstep any boundaries but that you need to let the person know that their behavior is harmful. If the truth is more of a "gossipy" situation (for example, you saw the person's significant other out at a bar with someone else), then I'd stay quiet and just let nature take its course...then there is the "truth" that is more of an opinion...it's kind of difficult with that one....my sister once had to tell me that I should consider waxing the hair on my upper lip (although blond, she said she could see it from a particular angle)....it didn't hurt my feelings at all but I left my lip alone since it just didn't bother me... :-)
