I feel uncomfortable around old friends, but I am okay around people I don't know. What could help with this? I tend to isolate myself because I feel so uncomfortable around my friends. Is this social anxiety?

Well... there's no history with people you don't know... & few expectations... especially if you don't expect to be seeing them again much... or ever. What could help? Perhaps it's just a matter of spending more time with people who make you feel uncomfortable. It may be just a matter of exposing yourself to what you find discomfiting.
