This will sound ridiculous but I find it difficult to compartmentalise between sex and love.

I feel that if I love someone my desire for sex with them decreases as I then feel I am taking advantage of a very, very good friend just for my own physical pleasure.

Like I say, it sounds absolutely ridiculous, I know full well that women enjoy sex just as much as men but I just have this stupid view that having sex with them gives them the impression that I don't actually care about them and that the only reason I am with them is for my own personal and selfish pleasure.

Really embarassed to have admitted that, but oh well.

Sex is a normal part of a romantic relationship so there shouldnt be any guilt. Maybe there is another issue attached to this that you and your T could explore

Something similar was described in Anna Karenina - towards the end, as Vronsky's love for Anna decreased, his sexual desire for her increased.
