I'm exhausted. I haven't healed correctly. I've been prescribed two antibiotics for a skin infection. I'm just spent. They would not let me call in sick to my 2nd job despite seeing the prescriptions.

No amount of money is worth this and neither is their discount. Should I give notice or just say see ya'?

I would always give notice when possible to do so. Never know if you might find yourself desperate enough to go back, or in need of a reference. Especially if you live in a small town, it's good for your reputation.
