Is it appropriate to put "wanted to kill self after starting" or is there something else that works better?

Geez. I had to have a big talk with my gp today about how i dont want cancer treatments because i would rather be unalive than sick. (They want to biopsy some calcifications.) I only saved myself by telling him about all my wonderful friends here and how we have imaginary holiday dinners together. "Reason for Leaving"
