Is it appropriate to put "wanted to kill self after starting" or is there something else that works better?

Geez. I had to have a big talk with my gp today about how i dont want cancer treatments because i would rather be unalive than sick. (They want to biopsy some calcifications.) I only saved myself by telling him about all my wonderful friends here and how we have imaginary holiday dinners together.

something that works better for whom?
If the answer is "The employer" then that something could be a list of the three top-most reasons you felt that way. If the answer is "me," you'll have your answer after a few good nights' sleep. __________________ Major Depressive Disorder; Sleep Apnea; possibly on the spectrum Nuvigil 50mg; Effexor 37.5mg Wellbutrin 150mg; meds for blood pressure & cholesterol |

I'm trying to get into some state aid thing for getting back to work (which is fking stupid because I have trouble eating and risk killing other people everytime I have to commute somewhere which is basically everywhere) and it's asking for reason for last leaving previous places of employment.
Top three reasons were mostly because I realized I am a useless POS who can't do anything and I don't want to put that either __________________ [Insert thought-provoking and comedic quote here] |

Okay, seriously. Maybe you can say something like "personality conflict" the follow it up with why that wont happen again? Lack of life goals, problem handling conflict. Inability to persevere - which youve certainly shown us now you DO have.
Sorry for my weird answer earlier, i was stressed out about my upcoming dr appt. |

You don’t normally need to provide a reason. But if you need it for a record of sorts then you can say that you are leaving for health reasons. That wouldn’t be a lie. Or job demands are too overwhelming. Could also be “long commute”.
