Primary doctor means that this is the physician who takes care of you for almost all things. Here, in the U.S., we see a specialist if for any reason our primary physician is really not very adept at managing whatever the situation is that calls for a specialist. For example, I have an internist who checks routinely all the bloodwork and orders special tests. I see a psychiatrist about once every four months for medication and talk (about 25 minutes).
I'm sorry your country's system is not as effective as it might be. We've just had a new system instituted in this country, and I'm waiting to see what will happen after the next election of a president. I hope your country is able financially to clear up the problems you are experiencing as citizens for your medical help.
It sounds frustrating, to say the least.
