Quick answer:

IP - in patient
OP - original post and/or poster, or possible outpatient, depending on context
pdoc - psychiatrist or anyone who prescribes psych meds
t - therapist or therapy
gp - general practioner - but the British folk use a lot of different names

Idk - I dont know about a chart - my search doesnt work on my phone. Hope this helped!
