I dont know about your location but here in mine when an ambulance or paramedics comes they take notes. on the notes they write down who made the call, what the problem is with the patient and any considerations for the patient. sometimes that includes any discussions they have with the patent or person who made the call, they also know the patients vitals, appearance and behavior.

Im guessing according to what you posted they put your name on the paper as the person who made the call and the RSDR (riskier) and reason why they are marking patient as riskier is because your mother was verbally abusing them (mouthing off, yelling, refusing to co operate to the point where they felt she was a danger for assaulting them.)

my suggestion is if this continues to bother you contact your treatment providers, they will be able to help you locate a copy of the report and explain what the report means.
