I want to quit medicating myself. no one knows I do it, not my family or friends, not my therapist. I want to tell my therapist. I'm going to try this on my own, I just want someone to know about it. If a person is self medicating too much on prescription medications, not for suicide, just to feel better, do they have to disclose to family members or your doctor? I will suffer withdrawls, but it is safe to detox myself on this medication. I think I just want my therapist's support.

Confidentiality rules may vary somewhat from state to state, but generally there are only 3 times a counsellor can disclose info. 1. you are at risk of harming yourself, 2. You're a danger to someone else. 3. A minor child is being abused.
If you're seeing an MD, they may have additional reporting requirements. For example in Ontario, if an MD thinks you're alcoholic, and so could be at risk of drinking and driving, they are required to report it to the Ministry of Transportation, which will then suspend your licence for up to a year. In the US, unless you are a minor the only time HIPPAA doesn't apply is if you are a danger to yourself or someone else. they are also mandated child abuse reporters.

Presumably the doctor prescribing your medicine and the pharmacy filling it know how much you are using? Are you sure you are using too much or are you getting the prescription meds illegally. I would talk to my doctor, rather than just my therapist but unless you are a minor, I don't think either would report you for wanting to get healthier. What you have done in the past is not a problem; what you are doing now if their concern.
