I'm in need of a good check list that I can use to evaluate my ADD symptoms. My pdoc wants me to report on how my symptoms are doing while on medication. Maybe something that has a scale or a way of noting intensity. We are trying to evaluate how well the medication is doing at different doses and if there are any problematic symptoms that the medication isn't touching.

Any ideas on where to find something like that?

Checked Web MD but no luck. I'm thinking of just making up my own. I've found 2 very comprehensive symptom checklists for diagnosing that I can tweek to fit my needs. Odd but I would think that such evaluation tools would already exist.

You may want to get someone else to evaluate as well. IMO, it can be hard to detect differences in focus and so on when taking an ADHD med. Other people can sometimes have a clearer perspective.
