My boss called me today and I lost my job. I have been in the office only 2 days over the last month and have called in sick all the other days (used sick/vacation time) I doubt I will get unemployment since I was absent so much and was given verbal warnings.

The claim form asks what the last day worked was. Is that the last day I was physically in the office (two weeks ago)? Or was it last Friday, even though I was not in but was a paid using a sick or vacation day.

It is too late for this time, but if it happens again try contacting the unemployment office when you get that first warning.

Can you put both days on the form?

I think the last day you worked was the last day you were employed. The last day you had a job.

Thank you. I'm doing it online I should have said so I have to put one date. I was thinking it was the last day I had a job, so last Friday, but wasn't sure.

Last day "physically" in the office should be just that - the one two weeks ago - when you were physically there.

You should be eligible for disability, but it sounds like a long process. Others can tell you the process, but where I live, my pdoc said you have to first get evaluated by a county pdoc, who would also refer to your current pdoc.

wait - no nevermind i read that wrong sorry! i have to fill those out for ex-employees and they ask me last date paid for work i think.... so to make sure yours will match theirs. Put the last date paid - so Friday.
