Mine asks all of those every single time. Also always asks what the date is and who the president is, asks if I'm having hallucinations, lots of the same questions every appointment.

My asks about sui stuff as its part of the process for his company guidelines.

Anything else I have going I just tell him. I trust him so I have no reason to be concerned about being honest.

Yes. Every time. They are ways to measure if I'm improving or getting worse. I don't mind them and will usually just volunteer the info as part of my answer to the opening question about how I've been doing.

I have to complete a long questionnaire each time I have a visit and my pdoc ask me all of those questions. I have to see her every 90 days because she does not give me refills.

My doc usually assesses me on her own, sort of. I'm asked how I am in general, I start talking and from there I sometimes get a, "So you're not having feelings of suicide, hopelessness, etc.?" type of response while other times it is very clear that I'm not in danger but just to be sure I'm asked something like, "So you aren't feeling down, etc. just so we're clear."

Yes, every time except when I'm obviously hypo/manic. My pdoc asked more probing questions at this last visit since I've now been inpatient for suicidal ideation---if I have any thoughts of harming myself, what method(s) I might use and whether I have access to them, what I'm supposed to do if I feel suicidal. (Fortunately, the firearms have been removed from my home and my pills are locked up, and my husband watches me fill my med minder each week then puts them away so I can't OD.)

I fill out a PHQ-9 form every time... and then we usually discuss the bigger topics. I've always tried to answer the questions with the first thought and tried to be as honest with myself as possible. I could never do this with someone I actually know though.

Damnit.

I see my pdoc tomorrow.

Long sleeves tomorrow. I went a bit psycho the other day and it's kind of obvious!
