I guess the logic behind that is to not be triggered and start drinking/using. To break the cycle of habit.

From my experience being in recovery, it does for a lot of people in AA and NA. I had to leave New York and everything behind to find a new life in North Carolina. I am at three years sober, and not looking back.

Yes, but it's not necessarily moving out of state or associating with different people. It's changing where you spend your time (not bars, liquor stores, that dude who's always drunk's apartment, crack shacks, etc.) and spending time with people who are and have been in recovery for long enough that imminent relapse isn't likely.

For myself I just changed my behavior. My husband still drinks in front of me. But from what I understand that would trigger most in recovery.
