If someone I had on ignore changed their name, would that cause them to become unignored?

not as far as I can tell. anytime anyone on my friends list and ignore list changes their name they remain on my friends list and ignore list but as their new name.

the only time someone on my ignore list did not show up as ignored was when they deleted one account and made a whole new account.

No, it shouldn't affect it at all. The name change is done through the vbulletin program so all things associated with and to the user should stay the same.
