Do you think we might be able to get some occupation-related smileys? Like a nurse, chef, baker, police officer, fire fighter/EMT, doctor, psychologist(?!?), book worm or librarian, others???

Definitely some doctor & therapist smilies & angry ones, & puzzled ones (docs & ts that is) & maybe a disbelieving version too.

How about the therapist on a chair and the client on the couch? Not to be a cliche, but would definitely be recognizeable...
