I am sorry that this happened to you and to your friend, Milly. You are not a bad friend because you didn't know. I am sure that she must have gone to great lengths to keep these things from you; this is often the case. It can be very, very difficult to disclose these things, even to a close friend.

Please give yourself acknowledgment for having been her friend during what must be a very tough time for her. Even though she wasn't able to disclose, I am sure that she got a little boost and some comfort just being with you and knowing that you were her friend.

Bull. You are not a bad friend, I consider you a friend and I think you're wonderful.

You know what cutting is all about, and is not your fault that that is what your friend is doing to cope. People can't be forced to stop, and you can try to be there for her to talk to, but that's about it. She's lucky to have a friend like you, someone who cares so much.

I'm sorry about the abuse she's suffering/suffered. Has she told anyone in a position of authority?
