I will make this one multiple choice! I will give each individual question a number, and select the one answer with that number that suits you best. Feel free to elaborate by posting comments if you want to as well.
Thanks for this!

No real reason. In my termination session with ex T last month I had to fill out a whole lot of forms with questions like these and compare them with my past answers to measure progress. When we were filling out the questionnaires together i found it interesting that some answers I went to choose "mild or moderate" but t said I should check "severe". It just got me thinking about symptoms of dissociative disorders over time, like some people here may have struggled with certain areas in the past but not now. So I am hoping there will be some answers to that affect!
But I am no great poll maker so it probably won't show much of anything really. To look at it seems pretty confusing really.
Thanks for this!

That's interesting that your t said you should mark some things severe when you saw them as moderate or mild. I just answered the poll, and I'm thinking about my answers. My last t had me fill out a short questionnaire every few weeks and she said the same thing to me a couple of times. I think I should have marked relationship problems as severe. I guess it's easy to get used to a certain level of disruption and then it becomes the norm. But, compared to a year ago I'm doing much better in all areas, also compared to a decade ago.

Relationships was one of the areas my T said to tick "severe". Another one was work and study because I had to leave my last job and have had to reduce my hours for my current one because I just can't cope with it. But I can work and I know some people can't at all so that doesn't mean "severe" to me. I think you are right that we are just used to a certain level of dysfunction. I think my "normalometer" is broken!

the last three i answered yes to, but they really all come and go, so i don't know really what 'currently' means because there can be weeks without them.

I would call that "current" as well. I suppose I think of "not current" as a year or two ago. But if you still experience those things sometimes I would definitely say it is current.

Looking at this poll blows my mind. There is a lot of suffering, isolation and emotional pain represented on there.
