9)When troublesome thoughts come like negative thing s or self harm thoughts I will attempt to block that from my mind and reassure myself everything will workout in its own time and God is the person with control over life.

10)In the event none coping skills work and i self harm(no one is perfect we all slip sometimes ) ,and i self injury i will do all the necessary things needed to clean the injury and bandage it so it wont be tempting to hurt it again (someone gave me that tip who self harmed alot).

12) Attempt to eat at least once per meal and build up slowly ,will distract right away before thoughts have a chance to influence me so breakfast (light)and lunch lot of carbs/protein/dairy/fruit,veggie choices ) and dinner whichever dinner is for the family.

13) Agree to send a copy to team if i come up with a suicide plan or self injury event to damage body. With details like who where when why.verbally agree to it (I......) GIVE MY FULL CONSENT AND PROMISE TO FOLLOW THIS WITHOUT QUESTIONS OR RESISTANCE)

14) IF I DANCE I AGREE TO CONSULT WITH DANCE INSTRUCTOR PRIOR TO STARTING and follow what she sets such as limits and such or recommends .

15) For time being not having self harm objects near till able to control my emotions and thoughts.

That's is a fine list that should be really helpful to you. I like how you are so pro active in keeping yourself safe and also mindful that you are human and do slip up sometimes as humans all do. But are going to handle any slipups with care to you.
Hope that your contract agreement works out well for you and helps you to feel better too.
