I am asked to fill out a counselling contract form and T says a few words of her own. Thats it.

No form, ever. My T wrote some notes in our first meeting and gave me a written copy of his cancellation policy.

I like filling in forms but would be scared of personal ones at the start before I knew and trusted him. So I'm glad there weren't any.

It was long and I was caught off guard because it asked me a bunch of questions about mental health symptoms that I was expecting to disclose in person. But once we got into session and especially after the first one, I was alright with it.
