Don't you present it at work to get days off? Rather than to a t? My mom did it when she had her colon surgery and recovery. Her surgeon and oncologist sent paperwork to GP and GP put it together for moms work, she took it there, well we took it there . She doesn't have t but If she did I am not sure why therapist would be presented with paperwork? Are you getting time off due to mi? Still wouldn't it be pdoc? Not t?

Back in 2009 when I took some time off work I got the paperwork from my job and took it to my pdoc and he filled it out and sent it in. Back then I was too much of a mess to even think about getting a t although my pdoc kept suggesting it.

At my internship patients sometimes need it filled out and it's no problem. We just inform them that we have to fill in the diagnosis and one of our medical staff has to sign off on it (nurse practitioner or doctor).

It turned out not to be a big deal. I was embarrassed about it but she did not make it into a big deal.
