Hello any Saskatchewanians! Or other Canadians.
I have a very lengthy form for my psychiatrist to fill out (gosh I expect it would take him just an hour alone to read the directions). If you've had to have your pdoc/GP fill out a form was there a general fee or do they charge by their time? (my pdoc is away so I can't ask). Thx a bunch |

It's going to vary somewhat by what your provincial health insurance covers re filling out forms, and the complexity of the form. My Dr. (I'm in ON) has different flat fees for stuff that's not covered by OHIP. But those fees are set by the Ontario Medical Association.

Im from Alberta, my psychiatrist has never charged me when i had to apply for disability and it took her more then a hour. My Gp has charged me $50 a page. I would google provincial health care card coverage and check it out. Every province is different on whats covered and not covered. Sorry im not much more help.

I'm in Ontario. I have never had to pay for medical forms. When I need one filled out I book an appointment and tell the office staff. My psychiatrist or family doctor and I have always filled them out together.

thanks everyone! I finally got through to my pdoc's office. He will be back and I have an appointment late next month. The office assistant said he would discuss cost with me then. If this is per page, I'm screwed,
