Only once. The client before me client cancelled right at their appointment time, so T called and told me to come up when I get there (I always arrive early and wait in my car till 5min before my appointment because I don't want to see her clients). I got there about 20mins early, and she ended at our normal time.

She sort of extended our session one other time. She and my H triggered me so bad that it wasn't safe to go home. Luckily, she didn't have a client after me, so she did another session right afterwards. I did have to pay for the second session as well, so I don't really count that as going over.

I'm glad my T stays on time. She's never late and she ends on time. It's stabilizing/comforting to me. I hate change and inconsistency.

Almost every time (by 5 or so minutes)... but to be fair, we usually start about 5 minutes late.

Yeah, it's strange - after starting therapy with this super-exact time frame I also became more mindful of time in my life in general. Previously I tended to be chronically late and I thought that I'm just such a person. But now for years already I tend to be either a bit early or exactly on time everywhere.

Same. Other than some parts of my work life that are very rigid in the "early is on time, on time is late" sort of way, I rarely notice or care about a 1-2 minute variance. I consider my therapist very prompt and reliable, but it's not unheard of for the session to start 1-2 minutes "late," which is completely fine with me and which I would count as starting on time.

Only one time. We were wrapping up, then I started saying something, but I stopped myself in the middle of it and said, "Never mind, I'll just get to it next time." But she said, "No, go ahead." I was pretty surprised. It was only about five minutes over time. I do think she often started wrapping up a little early anyway. By the end, I'd stomp out early every time anyway to avoid the wrap-up.
