For later this month and I got a questionnaire thing to fill in, problem is it's through work (the help I'm getting) so for some of the questions do I be economical with the truth or do I be completely honest answering the questions on the bit of paper they want me to fill in?

I vote for Economical since it’s thru work. You can decide on providing (or not) anything else if they ask.

Maybe I'm paranoid but I would only write down what is absolutely necessary to answer the questions and nothing more.

Is it like an employee assistance program? I would honestly vote for being as honest as possible, BUT I would also enquire about confidentiality before proceeding.

It's mental health support through work, counselling etc. They said they won't tell anyone anything that's said in session unless anyone is in danger of getting harmed.

Sounds like you know not to admit to self harm thoughts--especially on paper or with someone new. It takes a long time to know what can be talked about--there are several stories on here were admitting these thoughts have led people down the road of being committed for observation, etc.....

No, I would not give that information out, especially for anything connected with my job.
