I have worked with my psych np for about 10 years. First the first 5 or 6 years so worked in the same facility I do then she joined a new out of town practice and I followed her. When we worked together, it worked well because I could easily contact her whenever I needed to. She knows me and trusts my opinions on medications and if they need changes or not. She also knows that I struggle taking medications and will take them when needed but when things are going well I decrease or stop for a while. When my mood is an issue I will start back up. She wishes I would just stay on then and encourages me to do so but she knows that I know my body and have never put my self into a situation where my depression gets out of control. She also knew that T kept a check on my mood. She doesn't push when it comes to my meds because she knows if I don't feel like I need the medication or don't like one I won't take it. Again she trusts me.

For a while now I have been on a low does of my wellbutrin (150mg). I have in the past been up as high as 450mg. When I saw her about 5 weeks ago we discussed me going to 300mg. I usually start and stop without involving her. However, recently she has asked that I call her when I want to go back or increase not to just do it on my own. So I agreed that when I was ready for the increase I would call her.

I increased last weekend thinking it would be a few days. I called her office this Thursday. I had to tell her associate what I was calling for (to increase my medication). He started telling me about how she works. I explained I had been seeing her for a number of years so yes I know how she works. He said oh so you know she won't approve an increase without an appointment where she sees you in person. You have an appointment next friday so I will give her the message so that she knows that you need an increase. UGHHHH. They have always given her the message and she calls me back. People that is how WE work together. I never heard back from her. So I have stayed with the increase. On top of it all, this time last year she had a brain tumor and she was about for a couple of months and I had no idea what was happening.

I sooo feel your frustration! I say do what you normally would - the heck with the associate.

Hopefully if you talk with her at your session she can fix things with the office so that they handle it better next time. I had a nurse that worked for my first Pdoc that constantly interfered with a similar arrangement with me and Pdoc. Pdoc insisted that the other nurse (Mary) be the only one to deal with me because Mary usually understood what was going on but even if she didn’t she knew how to handle it in a way that didn’t set me off.

Sadly there seems to he a constant turn over of staff
