My doctor recently changed my dx to Schizoaffective. What does that mean exactly?

What was your diagnose before? If it was schizophrenia, it means your doctor thinks you have a mood disorder (major depression or bipolar) that goes along with your psychosis and sometimes it doesn't. If that makes sense.

Schizoaffective is a combo of elements of schizophrenia combined with elements of either BP or MDD. Since yours is bipolar, it would be a combo of schizophrenic and bipolar symptoms.
