For the past month, my depression has been in full swing and it’s been affecting my productivity and goals at work. This past week, my anxiety has been very hard to control due to major stress in my life and my only relief is taking a walk outside or restructuring my thoughts to calm myself down. I can’t stop the crying and try my best to hide it.

I am one of the top performers on my team and I haven’t been spot on with my work lately. I work in a corporate office and don’t know whether I should bring it to HR or just go directly to my boss. He and I have a good rapport and he is very open to any concerns of his employees. I want to take the initiative and make him aware that my overall performance is lacking because I don’t want our numbers to slip. My only hesitation is whether I should even talk about my personal life because I have a strong belief it should not interfere with professional environments. How and should I address it?

If you take this issue to your boss or HR, what outcome are you looking for? Understanding, time off, reduced work load, etc. I think it would be important to know what you expect to get from any discussion you have about this before you take it forward.

This is a tricky personnel road to navigate ... it is good you are giving it consideration before you have any discussion about it with management.

I agree with what Little Lulu said. Unless you are requesting some sort of accommodation, it may be best not to have this conversation at all, imo.

Plus, a lot of times we are harder on ourselves. You may know that your work performance is only at 92% instead of 100%, but other people around you are probably too absorbed with their own worries to really notice.

Maybe seek for personal leave of absence due to health issue. Consult a doctor, the doctor might be able to provide required papers to get you off work. Maybe you can qualify for absence with pay by having the doctor's request. Just a thought in addition to Little Lulu and hvert's comments.
