The *number* listed is people who've friended you.
Underneath that, if you have added anyone to your friends list/address book, it will show your friends online (if any). If you have never added anyone to your address book, you will never see anything in that box. If you have added people to your address book, and that person is online, you'll see their avatar listed there.
