Just need some opinions on how you feel about your medical records being correct. I'm not talking about a professional giving their professional opinion. I'm talking about lying about what they said to you, what you talked about, giving you paperwork and they didn't , about time spent with you, typing their entire report while you are talking to them, giving you the impression that they think you are someone else.

Would this be a serious issue for you?

This would be a big issue for me. I know as humans, we can all make mistakes, be forgetful, but lying would be a complete deal breaker for me.

That's the way I'm feeling. And this is just the short version that clients see. It's not one of those things like she added a diagnosis I didn't want on my records or something, that's a professional opinion. She asked if I was still taking a medication, and I told her I never took , nor was prescribed that medication and it didn't phase her a bit. Then she asked what happened the previous week , a couple of times and I had not idea what she was talking about. Last time I saw her was 4 mos previous and she made a comment about how "very sad" I looked then put in my records that I was less jovial than usual, what the heck?

She didn't ask about my medications, the last visit 4 mos ago was our first session, and she said the next visit we would discuss changing the medications or dosages, she never mentioned that. The list goes on and on.I think she thought I was someone else and that concerns me especially as medications go.

But on the other hand I don't want to get her in trouble. At the hospital I worked at they would fire someone for that. I don't want her to lose her job , but I don't want to see her anymore. I sent a letter to her by US Mail, instead of the "secure messaging" email service from the VA which is not secure at all. everyone I see at the VA has access to my charts as well as their nurses. The letter express my concerns and that I mostly disagreed with the jovial comment and to call me to discuss it. Not a word, nor has it changed. The chart may be locked but we could always put an addendum in if something needed to be added. No addendum. I've cancelled my next session with her. Not sure if the VA will let me continue without a Psychiatrist, but they might if I refuse the medication. I pick up the form (change in providers) today and would like to fill it out while there. But explaining why will be hard.

Yes, maybe she did think you were someone else if there are that many discrepancies.

I think sometimes it can be hard to be believed when dealing with "professionals", like they think that we can't think for ourselves, or that we are the ones who make the mistakes.

How much detail do you need to give about the reasons for wanting to change providers? Can you say it just wasn't a good fit and wasn't helpful?

Yes, I think that would be the best option really. Maybe by my bringing it to her attention she will be more careful with other clients. But I certainly can't trust her now. What gets me is when they ask for honesty and feedback from us it's expected to be done, when you ask the same from them they ignore it.
If she had just talked to me about I may have stayed with her and given her another chance, I mean that was only our second time meeting.
Thanks for the feedback though. When I get the form I think I will just make it a bland response
