Just need some opinions on how you feel about your medical records being correct. I'm not talking about a professional giving their professional opinion. I'm talking about lying about what they said to you, what you talked about, giving you paperwork and they didn't , about time spent with you, typing their entire report while you are talking to them, giving you the impression that they think you are someone else.
Would this be a serious issue for you?

This would be a big issue for me. I know as humans, we can all make mistakes, be forgetful, but lying would be a complete deal breaker for me.

Not sure if the VA will let me continue without a Psychiatrist, but they might if I refuse the medication. I pick up the form (change in providers) today and would like to fill it out while there. But explaining why will be hard.

Yes, maybe she did think you were someone else if there are that many discrepancies.
I think sometimes it can be hard to be believed when dealing with "professionals", like they think that we can't think for ourselves, or that we are the ones who make the mistakes. How much detail do you need to give about the reasons for wanting to change providers? Can you say it just wasn't a good fit and wasn't helpful?
