Was denied drivers license in new state. The name on my birth certificate was different from my SS# and old license. I changed my first and last name on SS# and license over 35 years ago. Never a problem until now.
I had not been to town in a couple years and was just starting to get the courage to go out. Now my license is expired. My husband is on meds for depression, anxiety and back pain for years, so all my attention has been on him. Very hard since I barely care for myself. Now I am really stuck. where do I even start? |

Hello, Iamwho. When you changed your name, did you do it through the court? Often you have to publish a notice of the proposed change in the paper and have the court sign an order approving the change. If so, my suggestion is for you to call the office of vital or public records in your state to find out the procedure for showing the name change of record. You might need to file a certified copy of the order for the name change and an affidavit. A certified copy of the birth certificate and proof of name change should be enough to get your license back. I suppose the motor vehicle department may require you to get tested again.
When you applied for your social security number, did you have to provide a birth certificate and proof of the name change? If so, you may be able to get a certified copy of what is on record at SSA if need be. Good luck. |

Thanks for your reply Byzantine. In 1975, after getting married , the DMV and social security let me change my first and last name just by filling out a form. I will see if SS has a record of this. Seems to change legally I need proof of ID for my original name. That was 35 years ago when I was 19.
This all seems impossible now. |

Iamwho, can you get a copy of your marriage license? there should be some record of your original name and your husband's name there. Do you have any copies of transcipts from school? Any of those might be usable, I would think.

Thanks for your reply lonegael. Will work on getting this information.
Rohag sent me info on 211.org and I found a legal aid source. I need to work on leaving the house to take care of this. Not doing well at taking care of anything right now. |

Still trying to resolve my legal ID problem.
I have e-mailed and sent letters to the main SS office. I Sent them the fee I was told to send for a certified copy of my original SS. I got a letter stating I needed to send a different fee. I sent them another check and I clearly stated it was for a "certified copy". They sent me a copy but it wasn't certified and had my birth year wrong! ![]() It was a year off and I didn't recognize my signature, but that was in 1970. I have sent SS a new request and another check to get a certified copy. Legal aid is looking at my case. Hopefully a solution will be found soon. Last edited by Iamwho; Jul 28, 2013 at 10:14 PM. Set backs like that are certainly worrying. Is that name changing the thing that burden you the most? Is there any else specific things?

Yes, there are many other major things I am dealing with. I recently had to go see my MD for a physical problem. He also started me on 20 mg Prozac then later added hydroxyzine 50mg (as needed). Don't know yet how well it is going to work. I finally did get a certified copy from social security showing when I changed my name. Still working on getting my identification straightened out. |
