I have felt like **** for a good 4 / 5 years now, but its all kind of come to head in the past year, things are getting worse and i just don;t no what to do

Can you tell us what kind of feelings you are having? If you think that your feelings stem from your depression, you need to see a therapist if you don't already have one. Getting a good physical is also something to think about. Either way, if you can get to the bottom of where you feelings are, you will have started on the road back to feeling better.

Is there a lot of stress in your life? Or perhaps a lot of things that you feel powerless to control? Is everything going fine but you just feel bad?

I think trying to determine if it is environment or biological could help. Is there one anything that make you feel depressed? I reccomend talking to your doctor and a mental health profession if things are really bad. If you are religious sometimes members of the clergy do counseling.

Try exercising and drinking more water. Also, try to talk to someone about your emotions. Talking often helps a person to feel better. I am always here if you need to talk.
