Not sure where to post this but would appreciate feedback.

I have to get two health forms completed for a school clinical program I've just started. I CAN think of reasons and know why they need the mental health information and meds but at the same time feel that privacy is being violated and freaking out. They don't know me yet and they will see the anti-psychotic among other things and I am worried about repercussions of judgments (as I am basing one now ironically). The fact that I am already stressed, I know is playing into this but I was hoping to start on a level playing field with this new program and it is a small community that I will be working in and with. No one knows and did not want to be known based solely on these reasons or act as a background in their minds.

One form is to be completed by the physician and I'm not as worried about, as my doc is excited about it the program. The other is completed by me and one of the questions states:

1. MENTAL HEALTH:
Have you ever received treatment for a nervous condition or emotional problems?
"Yes" is the only option
WHEN? then fill in the blank so "On-going", "Currently", what would be appropriate, suggestions? I can't get into listing dates as has been going on for so long but it will continue as I will always be treated. It doesn't ask 'for what' thankfully.

2. Then it wants a List of all meds, specify. I am tempted to put that "There is an up to date list in my wallet," as there is. LOL. Then I saw the doc is required to list them too; you should see how small the space is for both of us. They definitely won't fit.

I know it's not just about the forms, it's about the 'being known' part. My experiences to date have been treated like a pariah so have learned to try educate where I can but have kept it to myself until the right moment comes. I just don't know how to handle this. In the meantime I will try to calm down, but feel so disappointed and filled with dread. I can't get into T until after these forms are due, but will talk about this next time. Meanwhile, I really appreciate y'alls thoughts and support.

fresia, others may disagree with my opinion and i'm not sure what the program is about. when i had to fill out forms for my career employment i omitted my mental illness and meds, etc. even tho this info is considered private i felt i needed to protect myself due to ppl's lack of knowledge about my dx'es. i don't usually lie on purpose but that was my solution. another question i had re this-ever been treated in a psychiatric setting?, etc. i said, no. i added there is nothing that would prohibit me from completing the tasks of my job...in your case the program. you could also discuss with your doc re the med question. seems as tho he would/could omit the info too.
hope this helps you. i'm sure others will offer suggestions that may help you too contrary to my opinion perhaps.

You could include the information and hope for the best. Is this a mental health clinical program? In any case they ought to be sensitive to suggestions of stigma; if they are not, they need education. It might be good to talk this over with the doc first, but my present inclination would be to try to do what is "right" and let the chips fall where they may.

I agree with pachyderm. I would say: "Treatment is ongoing," and "See list." My thought is to attach the doctor's list, make sure it is complete and sign it.

I work in a role where we process this type of form. My internet is down, but should be back on 2mrw, so can write more then. I appreciate your concerns and you may want to be cautious what u write, but I wouldn't advise putting down inaccurate info in case it comes to light later and then you are viewed to have been dishonest. Msge me to remind me if you want, in case I can't find thread. And try not to worry, just a formality usually to make sure those who need it have support. SD
