This Monday I have a conference meeting with my mental health team, my doctor and the new mental health team I am being transferred to, my current team are totally useless, they have failed to help me in anyway, below is a list of questions I have compiled to ask the old team before they dump me on the new team, ps CPA is Care Plan Approach

1. Why did it take 3 months for my CPA interview to be scheduled and was only scheduled after I stopped taking my meds, only for nothing to be done in the last few months in regards to implementing any of the agreed items on the CPA?

2. Why have you not referred me to BCAS, ( alcohol referral team) even after I asked for a referral from you at the CPA meeting, it was agreed to be part of my CPA

3. Why have You not returned calls or answered emails sent by my primary doctor in the months before and after the CPA was arranged.

4. Why have you not sent my primary doctor ,or me, any telephone contact details in case of a emergency, or a copy of my CPA to my doctor or myself has agreed in the CPA interview.

5. Why have you not given my primary doctor a list of the new medications agreed at the CPA meeting

6. Why have you socially isolated me and left my to hang for the last 7 months waiting for you to contact me for appointments that are never made, and when they are it is to fill in housing benefit forms to cover your own asses.

7. Why was I given a inept social worker and not a community psychiatric nurse to work with me.

8. Why was I given a emergency meeting with said social worker only to be given medical referral forms to fill in at home, that could, and should have been completed months ago by any competent social worker who bothers to look at my file where all the information required is stored.

9. Why am I still in emergency accommodation after 7 months after I asked for help getting on the locator list to be permanently accommodated, something else you haven’t done anything about even though it is listed in the CPA meeting

10. Why did it take 5 months of me asking, I asked for a private phone line the day I moved in, for you to contact the landlord/slumlord to get authorisation for a private phone line to be installed in my room so I could contact my online support groups from the death trap I live in.

11. Why did you put me in accommodation that is a danger to my health,
no fire extinguishers or fire blankets in the kitchen,
smoke alarms are non existent in the rooms, or not working correctly in the hallway,
light fitting hanging broken, with wires exposed, from the ceilings in the hallway, my room
only 1 exit in and out of the building and that is a confined flight of stairs with no hand rail,
if there is a fire downstairs the only way out would be to jump out of the window, which is double glazed and doesn’t open fully enough to climb out,
If I do manage to get out the window, I have to jump onto a corrugated iron roof below my window 20 foot below me,
Sink installed in my room over 2 months ago yet still not connected to water supply, yet rent has gone up

12. Why after my last “visit” to your offices on the 30th of July was I not told I was being transferred to another mental health team by the social worker even though she new I had a letter in the post regarding this, which arrived on the 31st of July, also why has she not contacted me has arranged on the 30th of July, in regards to the medical assessment forms she tried to palm off on me, that’s something else you have failed to do.

In short you have systematically failed me at every step of the way, your support has been non existent, you only “act” when I pointed out to my primary doctor that after 4 months I still didn’t have a CPA, I have been placed in no-mans land in regards to liveable accommodation, or even the chance to move into liveable accommodation has been delayed by your lack of action in regards to filling out the necessary paperwork, this is the second time you have tried to palm me off to another mental health team, your uncaring, unprofessional, unsympathetic approach is beyond contempt.

That's quite a listing mellors. Wow. I know the feelings but along a different vein. I hope you have been able to feel better about it now that you have it all written...and can work it out of your system. Also, I would hope that you can reread each one, and view it as though it were someone else who wasn't so angry ...and edit it some.

Odds are they won't "get it" anyway. What do you hope to accomplish with giving this to THEM, as they didn't bother to do these items, and won't be around to correct them now? Is there another person you could address these issues with instead?

Either way... I hope you can work through this with your new team, and be safer. TC!

the whole object of the list is 1 to chew out the old team on there departure, they know this is comming, and 2 to let the new team know what problems i am having so they can make a start rectifying them,

enough is enough i really dont need this stress off the people that are supposed to be helping me and by putting it in writing and giving the list to both teams in the meeting i am hoping that my concerns WILL be addressed this time, also by giving the list in writing to my doctor ( who is brilliant by the way) she will put a copy on file in my medical records making it virtually impossible for the new team to do the same thing to me.

this was not my first draft of the list, i had to cut the swearing out and reword a few lines, the original list was 2 full pages long, if i had left it all in then i know they will not do anything about it, just file and forget, that is one option i am taking away from them
