i was wondering if i could get a printout of everything they have on me at the clinic? so i can cross-examine it myself ? they never gave me any papers with a Dx or anything... would be easier to explain some things to my family if i had a "doctors note"

Not certain about clinics, but the form that my neuro/pdoc office uses, is called a Summary of Visit Form. States meds, directions for meds, and my dxs, with billing codes, along with future appointments. Maybe, they have similar?

Here in Canada you ask for a copy of your medical record at any given office, and you pay "photocopying"which is about $30. You probably have something similar. It doesn't hurt to just ask for it. Then you'll know if there's a charge or not. The hospital always gives me a copy of surgery stuff with no charge. Always good to have a record of what they've done.

yeah when i go in i normally dont leave with any paperwork unless i see the pdoc.. which is like once every 6 months or so i will just ask about it when i go in next week and hope that they aren't "threatened" by it, or think that im going crazy on them im hoping to be able to see my case manager monday or tuesday for a couple of minute to talk about a few of my concerns.. i just would like to have the same records they have so i can comply and work better with them for a treatment that would actually help me
