If I went to have a psych evauation and he thought it necessary to be hospitalised and they did blood work and found that I have a narcotic med in my blood stream and they didn't belong to me, could I be charged?

Not unless you are pregnant. Otherwise it would violate confidentiality.

So say if you were my therapist and I came in to see you and you suspected that I was on something, what could you do about it?

Nothing legally unless I thought it had to do with child abuse, elder abuse or a factor in you threatening to kill/hurt yourself or someone else. I would challenge you on it personally though.
