My relationship with my current t may be coming to an end. I have an appointment with a new t for an assessment to see if a new approach may help move forward. Can anyone help me with what kind of questions i should ask the new t to see if we could work together

How well do you understand the "new approach"? Think of what didn't work with the old approach (don't know why you are coming to an end with the old T) and formulate questions that would ask about what you found unsatisfactory.

Attitude to email is something you should find out right at the start.
Otherwise it leads to all sorts of friction later.
