To me 30 pages sounds excessive for an initial visit. I had to fill out a two page medical history form and some hippa/privacy type stuff.
But the credit card stuff sounds sketchy. Storing credit card numbers on paper or electronically carries liability for the business storing the information. I used to work in an office where people could mail in ticket orders with the cc# written on the form for payment. The procedures for processing and then destroying those forms were very stringent. I pay with a credit card for t, but it is run through a machine like they handle it at a store. I would never agree to them writing down my number or storing it in their computer system. Perhaps this t's been "burned" by bad checks, fake bills or unpaid for appointments. If it were me, I'd keep looking for a t. If you decide to stay, I would ask a lot of questions about how your credit card information is kept secure. A reputable business will have policies stating how they keep customer credit card information secure.
