I don't think the issue in the vingette is that the therapist has a low skill level. I think the issue that the therapist practices CBT and the client needs an entirely different kind of therapy. The client is acting out and demonstrating an attachment disorder, which is not generally treated through CBT. Referring the client to a therapist who works psychodynamically seems to be in the best interest of the patient. This does not reflect negatively on the therapist's ability to practice CBT with clients who would benefit from that modality of treatment.
