I terminated T a couple of months ago.

Now I would like a copy of my records.

Would it be okey to write:

Dear xxx

I have thought about this for awhile, and would like to get a full copy of my records, while being treated by you from date :xxx to xxx

I would like for you to send all copies to my home adress. If theres any fee involved please let me know.

Your letter sounds good to me. You could add that you're entitled under HIPPA law, if you anticipate a problem. I wrote my T too asking for records but I was dismissed (I get that kind of reaction a lot). I had to hire a lawyer. Cost me way too much money $1,200. You'll probably get a better response. I don't think HIPAA applies to this situation in any way. It would if you were asking records to be sent to another person because of your right to privacy but I don't think this is anything like that.

I am doubtful that they will mail your records anyway. What is to stop your BF or a person who has ill will from sending a letter purporting to be you and want copies sent to an address that is not yours?

I want to ask him to send them to my home address. And I donīt have a boyfriend. I was planning on emailing the request from my personal e-mail, too.

If you just want to know what they say, ask to come in to review them. This way, ur T won't charge u for copying them. U can always take pictures of certain notes with your phone. My T allowed this. Good luck!

Thanks. I want some other T to look at them as I havenīt been able to get answers to my questions in the past. Looking at them with my former T isnīt an option.

If you want the records so that they can be reviewed by a new t, why don't you just sign a release to have them sent to the t? I do this all of the time between old and new t's as well as my pdoc.
