I thnk my T is using me as a case study at a conference without my permission

It's not a breach of confidentiality so long as names and identifying information have been omitted. I think most Ts would say something before using an existing client as a case study just for this reason. With that in mind I think it's very possible that, as others have said, your T drew a composite based on several clients rather than just one.
