My wife and kids. I would tell sibs and close friends if it comes up. I sort of did as my pdoc was therapy also at first. She is geared toward the behaviors that are generally responsive to meds and suggested I also find someone to work with me on root causes of thoughts and impulse that aren't easily medicated away.
Kids are glad to see me working on it. My wife seems to be pulling away; even though I am making an effort, it isn't the first time I had a substance issue. She seemed supportive at first then made a comment that she's "just done with all this" when I tried to talk to her about an aspect of my mental health. She is not interested in couple's therapy either. I would have discussed it with her in detail had she asked; now I am guarded. If she asks now I will question her intent.
The really tough part is that a lot of the things I say in therapy are about my rocky relationship with my wife. She is passive aggressive. I can't easily discuss this with her and I won't play that game with discussing it with the kids.
